ABSTRACT

Resumen Objetivo: Describir los tipos de cateterismo cardiaco que se realizan en el único laboratorio de hemodinamia pediátrica dentro de la red de salud pública de El Salvador, en colaboración con organizaciones sin fines de lucro. Material y método: Se realizó un estudio descriptivo retrospectivo de corte transversal en el periodo entre mayo de 2022 a enero de 2023, revisando los expedientes de todos los pacientes que pasaron a cateterismo cardiaco. Resultados: Se realizaron 54 procedimientos dentro de la sala de hemodinamia, 37 pacientes de sexo femenino. El peso fue de 20.6 ± 14.5 kg, de los procedimientos, 47 (87%) fueron terapéuticos y 7 (13%) diagnósticos. La oclusión del conducto arterioso se realizó en el 46% de los pacientes. No se reportaron complicaciones, los pacientes fueron dados de alta luego de 12 horas, todos los insumos fueron donados por fundaciones sin fines de lucro. Conclusiones: Nuestro laboratorio de hemodinamia realiza procedimientos de complejidad variada sin reportar hasta el momento complicaciones mayores. Nos vemos limitados debido al costo alto de algunos dispositivos, a la baja frecuencia de la utilización de estos y a la imposibilidad para adquirirlos en el mercado nacional.


Abstract Objective: To describe the types of cardiac catheterization that are performed in the only pediatric catheterization laboratory within the public health network of El Salvador, in collaboration with non-profit organizations. Material and method: A descriptive, retrospective, cross-sectional study was conducted in the period from May 2022 to January 2023, reviewing the records of all patients who underwent cardiac catheterization. Results: 54 procedures were performed in the catheterization room, 37 female patients. The weight was 20.6 ± 14.5 kg, of the procedures, 47 (87%) were therapeutic and 7 (13%) diagnostic. Occlusion of the ductus arteriosus was performed in 46% of the patients. No complications were reported, the patients were discharged after 12 hours, all supplies were donated by non-profit foundations. Conclusions: Our catheterization laboratory performs procedures of varied complexity without reporting any major complications to date. We are limited due to the high cost of some devices, the low frequency of their use and the impossibility of acquiring them in the national market.

ABSTRACT

Introducción. La pandemia por COVID-19 presionó los sistemas de salud para mantener alerta y activos los programas de control y prevención de las enfermedades transmitidas por vectores, y generó cambios en las estrategias de control vectorial en áreas urbanas afectadas por el dengue, el Zika y el chikunguña. Objetivo. Describir las adaptaciones del programa de vigilancia y control de vectores en Medellín durante la contingencia sanitaria por COVID-19. Materiales y métodos. Iniciada la emergencia sanitaria, se elaboraron protocolos de bioseguridad. Se fortaleció la vigilancia entomológica institucional en lugar de las viviendas. La información se recolectó en Medellín durante los años 2018 a 2021, en las actividades del programa de vigilancia y control de vectores, que incluyen la vigilancia epidemiológica y entomo-virológica, el levantamiento de los índices entomológicos, el monitoreo de ovitrampas, la movilización social y comunitaria, la búsqueda y eliminación de criaderos, y el control químico; estas acciones se adaptarons o incrementaron para favorecer, de una parte, el autocuidado de las comunidades en confinamiento total y parcial, y de desarrollar las acciones de prevención y control. Resultados. Se incrementó en un 40 % la vigilancia del mosquito mediante ovitrampas, la vigilancia entomo-virológica presentó un incremento de 34,4 % en el 2020 respecto al 2019, y se utilizaron herramientas virtuales para mantener y mejorar el contacto con la comunidad. Conclusión. La pandemia por COVID-19 causó gran impacto en los programas de prevención y control de las enfermedades transmitidas por vectores. Medellín adaptó rápidamente las actividades de vigilancia entomo-virológica, las acciones de control y la comunicación con la comunidad durante la pandemia, y esto permitió mantener activo el programa del manejo integrado de vectores en la ciudad.


Introduction: The COVID-19 pandemic pressured health care systems to remain alert and active in their vector-borne disease control and prevention programs, leading to changes in vector control strategies in urban areas affected by dengue, Zika and chikungunya. Objective: To describe the adaptations made to the vector control and surveillance program in Medellín during the COVID-19 health emergency. Materials and methods: Once the health emergency started, biosecurity protocols were developed. Entomological surveillance was strengthened from the institutional environment instead of homes. Data was collected in Medellín from 2018 to 2021 during the vector control and surveillance program activities, which included epidemiological and entomo- virological surveillance, entomological index survey, ovitrap monitoring, community mobilization, search and elimination of mosquito breading sites, and chemical control. These actions were adapted and/or increased to promote self-care among communities in total and partial confinement, and to develop prevention and control measures. Results: Mosquito monitoring was increased by 40% using ovitraps, entomological- virological surveillance showed an increase in 2020 of 34,4% compared to 2019 and virtual media was used to keep and improve contact with the community. Conclusion: The COVID-19 pandemic had a significant impact on arbovirus prevention and control programs. The city of Medellín quickly adapted its entomo-virological surveillance activities, control measures, and the contact with the community during the pandemic, which allow the Integrated Vector Management program to remain active in the city.

ABSTRACT

Background: The impact of SARS-CoV-2 infection in Africa is still unclear. In comparison to Europe and North America, morbidity and death rates are lower. Several factors have been proposed, including geographical variation in virus impact, environmental factors, differences in age distribution, and the impact of infectious diseases such as malaria, HIV infection and tuberculosis. Objectives: We investigated the clinical characteristics and putative determinants linked with COVID-19 in Angolan patients. Methods: Cross-sectional study undertaken at Military Hospital, Luanda, from March 2020 to March 2021. The survey collected sociodemographic and clinical information. Results: The sample included 1,683 patients aged ≥18 years, 64% men, with mean age of 46.3 years. SARS-CoV-2 was positive in 39% of the cases with RT-PCR. Patients ≥46 years with a level of education of ≥12 years had a considerably higher likelihood of testing positive. About 58% of positive patients had at least one comorbidity, of which hypertension and Diabetes were associated with SARS-CoV-2 infection. HIV and pulmonary TB were putative protective factors. About 14% of positive patients died. Most deaths occurred in patients ≥46 years, with less education and unemployed. Working as a healthcare practitioner was linked to a protective effect. Malignant diseases were the most common comorbidities associated with death. Conclusions: We identified putative factors related to SARS-CoV-2 infection and mortality. HIV and TB were protective and not associated with mortality. Further study with a broader scope should be conducted to explain the main features related to COVID-19 mortality in Angola

ABSTRACT

Resumen La ablación con catéter de taquicardia ventricular ha demostrado mejorar la supervivencia y algunas veces es el único tratamiento efectivo, sobre todo en tormenta arrítmica; sin embargo la presencia de inestabilidad hemodinámica dificulta el procedimiento. El soporte con oxigenación por membrana extracorpórea (ECMO) es una alternativa para lograr el mapeo y ablación de taquicardia ventricular durante largos periodos de tiempo. Se presenta el caso de un paciente con cardiopatía isquémica y taquicardia ventricular con inestabilidad hemodinámica, en quien se realiza la ablación exitosa del sustrato mediante soporte con ECMO veno-arterial, sin episodios de taquicardia ventricular durante dos años de seguimiento.


Abstract Catheter ablation is useful for reducing drug refractory ventricular tachycardia (VT) episodes and can be life-saving when VT is incessant or arrhythmic storm. Left ventricular hemodynamic support may be required in patients with VT and hemodynamic instability. Extracorporeal membrane oxygenation (ECMO) support is an alternative to achieve ventricular tachycardia mapping and ablation over long periods of time. We present a case of successful catheter ablation of substrate in a patient with ischemic heart disease and ventricular tachycardia with hemodynamic instability performed using venous- arterial ECMO support. There were not episodes of ventricular tachycardia after 2 years of follow-up.

ABSTRACT

The aim of this study was to verify the relationship between quantitative T2 relaxation measurements of lumbar intervertebral discs (IVDs) and spinopelvic parameters in patients with chronic low back pain. The study was approved by the Clinical Hospital of the Ribeirao Preto Medical School (USP) Ethics Committee, and written consent was obtained from all patients. A total of 455 IVDs from 91 consecutive patients with chronic low back pain were included in this prospective study. All subjects were assessed using the Oswestry Disability Index and visual analogue scale questionnaires and were confirmed to have no other spine diseases except disc degeneration. Spinopelvic parameters including the pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), sagittal vertical axis (SVA), global tilt (GT), T1 pelvic angle (TPA), lumbar lordosis (LL), thoracic kyphosis (TK), pelvic incidence minus lumbar lordosis mismatch (PI-LL), and lack of lumbar lordosis (LLL) were measured. The study group was categorized according to the Roussouly classification. Sagittal T2 maps were acquired to extract the IVD relaxation times, and the complete manual segmentation of IVDs at all levels was performed using Display® software. Lumbar IVD T2 relaxation times showed significant correlation with PT (P<0.01), GT (P<0.01), TPA (P<0.01), PI-LL (P=0.01), and LLL (P=0.01). No difference was noted between Roussouly subtypes regarding T2 relaxation times at any disc level. Data from questionnaires showed no correlation with T2 relaxation times. Global tilt and T1 pelvic angle were correlated with IVD composition changes (T2 relaxometry). There was no correlation between clinical symptoms and IVD T2 relaxation times.

ABSTRACT

The melanomacrophage centers (MMCs) in the liver of fish are indicators of environmental conditions, as they are involved in xenobiotic biotransformation. The objective of this study was to evaluate the number of MMC in the liver of juveniles and adults of Sciades herzbergii from areas with different levels of contamination. The fish were caught at three points (reference - A1, potentially impacted - A2 and contaminated - A3), in São José bay (Maranhão, Brazil), in four samples. The livers were subjected to the standard histological procedure and 5µm sections were stained with hematoxylin-eosin. In livers of A2 adult individuals (260.50±161.50 MMCs / mm²) they presented a greater number of MMCs when compared to A3 adults (60.00 ± 30.10 MMCs / mm²). Juveniles showed considerable values in A1 (100.00 ± 0.00 MMCs/mm²) and A2 (95.33 ± 33.00 MMCs / mm²) compared to juveniles in A3 (49.00±0.00 MMCs/mm²). These high values are unexpected for young people. The average number of MMC correlated with the rainy season in the region. The use of hepatic MMCs as a biomarker of exposure to pollutants, in particular substances from fisheries systems, such as ammonia and nitrite, proved to be adequate to differentiate areas with different levels of impacts.(AU)


Os centros melanomacrófagos (MMCs) no fígado de peixes são indicadores das condições ambientais, pois estão envolvidos na biotransformação xenobiótica. O objetivo deste estudo foi avaliar o número de MMC no fígado de juvenis e adultos de Sciades herzbergii de áreas com diferentes níveis de contaminação. Os peixes foram capturados em três pontos (referência - A1; potencialmente impactado - A2; e contaminado - A3), na baía de São José (Maranhão, Brasil), em quatro amostras. Os fígados foram submetidos ao procedimento histológico padrão e cortes de 5µm foram corados com hematoxilina-eosina. Em fígados de indivíduos adultos A2 (260,50±161,50 MMCs/mm²), eles apresentaram maior número de MMCs quando comparados aos adultos A3 (60,00±30,10 MMCs/mm²). Os juvenis apresentaram valores elevados em A1 (100,00 ± 0,00 MMCs/mm²) e A2 (95,33±33,00 MMCs/mm²) quando comparados aos juvenis em A3 (49,00±0,00 MMCs/mm²). Esses altos valores são inesperados para os jovens. O número médio de MMC correlacionou-se com a época chuvosa na região. A utilização de MMCs hepáticos como biomarcador de exposição a poluentes, em particular substâncias provenientes de sistemas pesqueiros, como amônia e nitrito, mostrou-se adequada para diferenciar áreas com diferentes níveis de impactos.(AU)

ABSTRACT

Resumen El tumor de células granulares es una neoplasia infrecuente, de comportamiento biológico benigno. Por lo general, se presenta entre la cuarta y sexta década de vida como una lesión solitaria, de crecimiento lento y buen pronóstico. Su histogénesis es probablemente de origen neural siendo positivo para S-100 y Enolasa Neuronal Especifica. Se muestra un caso con una localización inusual en la región axilar, las dificultades para alcanzar el diagnóstico puesto que puede confundirse con otras neoplasias, y los elementos clínicos esenciales de este tipo de tumor.


Abstract Granular cell tumor is a rare neoplasm with benign behavior. It usually occurs in the fourth to sixth decade of life as a solitary, slow growing lesion with a good prognosis. Its histogenesis is probably of neural origin, being positive for S-100 and Neuron-Specific Enolase. We demonstrate an unusual location in the axillary region, the obstacles to reaching the diagnosis since it can be confused with other malignancies, and the essential elements for clinically suspecting benign lesions of this type.

ABSTRACT

Introducción: la fístula dural arteriovenosa espinal (FDAVE) es una enfermedad vascular rara, de etiología desconocida y frecuentemente subdiagnosticada. El tratamiento puede ser microquirúrgico o endovascular. Material y Método: Análisis retrospectivo de una serie de 8 pacientes consecutivos con FDAVE tratados por microcirugía entre 2010 y 2020. Fueron evaluados parámetros como edad, sexo, cuadro clínico pre y postoperatorio medido con las escalas de Aminoff-Logue y Rankin modificada. Los estudios diagnósticos con RMN (Resonancia Magnética Nuclear), ARM (Angio Resonancia Magnética) y ADM (Angiografía Digital Medular) se utilizaron para determinar nivel lesional y resultados quirúrgicos. Resultados: Fueron operados 8 pacientes (7 masculinos y 1 femenino) con un promedio de edad de 58 años. El tiempo de evolución del cuadro clínico al diagnóstico fue menor a 12 meses salvo un caso de 32 meses. Las FDAVE fueron localizadas en: 6 a nivel dorsal entre D6 y D12, una en L2 y la última en S1 (5 derechas y 3 izquierdas). La arteria de Adamkiewicz se identificó en: 4 casos en L1, 2 en D12, 1 en D10 y un caso en D7 (6 izquierdas y 2 derechas). De los 8 pacientes operados, 3 fueron embolizados previamente. La evolución postoperatoria del cuadro neurológico fue: 2 de 8 permanecieron estables y 6 de 8 mejoraron uno o más puntos en la escala de Rankin modificada; no hubo complicaciones en el postoperatorio. Todos los pacientes mejoraron las imágenes en RMN diferida y la ADM luego de los 6 meses fue negativa. El seguimiento promedio fue de 48 meses con un rango de 11 a 116 meses, ningún paciente presentó recidiva de la FDAVE. Conclusiones: El tratamiento quirúrgico de las FDAVE es un método muy eficaz, de baja morbilidad y menor tasa de recurrencia comparado con el tratamiento endovascular.


Introduction: Spinal dural arteriovenous fistula (SDAVF) is a rare vascular disease, of unknown etiology and frequently underdiagnosed. Treatment can be microsurgical or endovascular. Material and Method: Retrospective analysis of a series of 8 SDAVF patients treated by microsurgery between 2010 and 2020. Parameters including age, sex, pre and postoperative clinical condition were analyzed according to modified Aminoff-Logue and modified Rankin scales. Diagnostic studies such as MRI (Magnetic Resonance Imaging), MRA (Magnetic Resonance Angiogram) and spinal DSA (Digital Subtraction Angiography), were evaluated for lesion level, as were surgical results. Results: Eight patients (7 male and 1 female), average age of 58 years were operated. The interval from symptom onset to diagnosis was less than 12 months in all cases except one (32 months). SDAVF locations were thoracic in 6 cases between T6 and T12, at L2 in one and at S1 in one case (5 on the right and 3 on the left). The Adamkiewicz artery was identified in: 4 cases at L1, 2 at D12, 1 at D10 and in one case at D7 (6 left-sided and 2 right-sided). Three of the 8 patients operated had undergone prior embolization. Postoperative neurological outcomes showed: 2 patients remained stable and 6 had improved one or more points on the modified Rankin scale; no postoperative complications were observed. Follow-up MRI images improved in all cases and spinal DSA was negative at six months. Average follow-up was 48 months (range 11 to 116 months), no patient presented recurrence. Conclusions: Microsurgical treatment of SDAVF proved to be efficient, with low morbidity and lower recurrence rates compared to endovascular results.

Descriptors were adapted to the interfaces of eight bibliographic databases were configured: PubMed, Ovid, SciELO, Ebsco, Scopus, LILACS, Epistemonikos and Embase. Results About 45% of the articles reported adult population, thirteen reported mixed population (adult and pediatric). 3 525 asymptomatic people were reported, with an average of 37,1 years [0.5-82 years]. Although the effectiveness of the control and prevention measures was not reported, the identification, isolation and follow-up of contacts stands out as a potential effective mechanism to prevent the transmission. Conclusions The use of this information could be relevant to guide evidence-based public health policies and the protection of populations and the improvement of health care that contributes to stopping this pandemic.

Se adaptaron descriptores a las interfaces de ocho bases de datos bibliográficas: Pub-Med, Ovid, SciELO, Ebsco, Scopus, LILACS, Epistemonikos y Embase. Resultados Aproximadamente el 45% de los artículos reportaron población adulta, trece estudios informaron población mixta (adultos y pediátricos). Se identificaron 3 525 personas asintomáticas, con un promedio de 37,1 años [0,5-82 años]. Si bien no se reportó efectividad de medidas de control y prevención, la identificación, aislamiento y seguimiento de los contactos se destaca como un potencial mecanismo efectivo para prevenir la transmisión. RESUMEN Introducción: Las enfermedades cardiovasculares aportan el mayor número de defunciones anuales en todo el planeta, por lo que son consideradas un verdadero azote para la humanidad; dentro de estas, cobra relevancia especial la cardiopatía isquémica por exhibir las mayores tasas de mortalidad universal. Objetivo: Caracterizar a los pacientes fallecidos con diagnóstico de infarto agudo de miocardio. Método: Se realizó un estudio descriptivo transversal con 158 pacientes fallecidos por infarto agudo de miocardio en el Hospital Provincial Universitario Arnaldo Milián Castro de Santa Clara, Villa Clara (Cuba), de 2013 a 2018. Resultados: Predominaron los fallecidos del sexo masculino (83; 52,5%) con edades entre 70 y 79 años (66; 41,8%); 104 fallecidos (65,8%) presentaron al menos cuatro factores de riesgo, con predominio individual de la hipertensión arterial (122; 77,2%), la diabetes mellitus (108; 68,4%) y la dislipidemia (99; 62,7%). La presentación fue típica en ambos sexos (masculino 39 y femenino 55) con localización mayormente anterior extenso y se realizó trombólisis en 51 casos (32,3%), antes de las primeras 12 horas de iniciados los síntomas. La complicación más frecuente fue la disfunción ventricular izquierda (42,4%). No se pudo evaluar la coincidencia diagnóstica en 40 casos, porque no se les practicó necropsia; pero se coincidió totalmente en 86 fallecidos (54,4%) y de forma parcial en 25 (15,8%). Se dejaron de vivir 2774 años y se obtuvo una línea del tiempo descendente. Conclusiones: La mortalidad por infarto agudo de miocardio mostró una ligera tendencia a la disminución de los fallecidos informados por certificado de defunción, con un promedio de 25,2 años de vida potencialmente perdidos.


ABSTRACT Introduction: Cardiovascular diseases account for the largest number of deaths worldwide each year; reason why they are considered a real scourge for humanity. Ischemic cardiomyopathy ranks high among them as it exhibits the highest global mortality rates. Objective: To characterize deceased patients diagnosed with acute myocardial infarction. Method: A cross-sectional descriptive study was carried out with 158 patients who died from acute myocardial infarction in the Hospital Provincial Universitario Arnaldo Milián Castro of Santa Clara, Villa Clara (Cuba), from 2013 to 2018. Results: Males (83; 52.5%) between 70 and 79 years of age (66; 41.8%) predominated. A total of 104 deceased (65.8%) presented at least four risk factors with individual predominance of high blood pressure (122; 77.2%), diabetes mellitus (108; 68.4%) and dyslipidemia (99; 62.7%). The clinical presentation was typical in both sexes (male 39 and female 55), large anterior wall myocardial infarction prevailed, and thrombolysis was performed in 51 cases (32.3%) before the first 12 hours of onset of symptoms. Left ventricular dysfunction was the most frequent complication (42.4%). No necropsy was performed on 40 cases, which did not allow for the assessment of diagnostic coincidence; however, there was a total coincidence in 86 deaths (54,4%) and a partial coincidence in 25 (15,8%). About 2774 years were not lived and a descending time line was attained. Conclusions: Mortality from acute myocardial infarction showed a slight downward trend in the number of deaths reported by death certificate, with an average of 25.2 years of life potentially lost.

RESUMEN Introducción: Cinco indicadores de salud cardiovascular: presión arterial, actividad física, índice de masa corporal, consumo de fruta y verdura y fumar tabaco, pueden ser suficientes para predecir el riesgo cardiovascular en individuos sanos a través del índice de Fuster BEWAT. Objetivo: Comparar los resultados de los índices de Fuster BEWAT y el de salud cardiovascular ideal para determinar la salud. Método: Se realizó un estudio descriptivo transversal en el servicio de Atención Primaria del Hospital Manuel Fajardo Rivero de Santa Clara, en el período enero-junio de 2019, se valoró el estado de salud cardiovascular de 347 individuos sanos, sin antecedentes de enfermedad cardiovascular con edades comprendidas entre 40 y 59 años. Se usó el índice de Kappa de Cohen para determinar la concordancia entre el índice de salud cardiovascular ideal y el índice Fuster-BEWAT. Resultados: La escasa actividad física (79,8%), la dieta no balanceada (74,9 %) y la presión arterial no controlada fueron los peores marcadores encontrados. Solo un 15,3% de los hombres y mujeres analizados cumplían con el estado ideal para todos los marcadores del índice de Fuster BEWAT y 17,8% del índice de salud cardiovascular ideal. La concordancia obtenida entre ambos índices, a través del coeficiente Kappa de Cohen (0.935), demostró una relación casi perfecta. Conclusiones: Las puntuaciones de los dos índices mostraron valores similares. El índice Fuster-BEWAT es más sencillo y no requiere resultados analíticos; puede considerarse la primera opción en contextos en los que el acceso a análisis de laboratorio es limitado.


ABSTRACT Introduction: The following five indicators of cardiovascular health: blood pressure, physical activity, body mass index, fruit and vegetable consumption and tobacco smoking, may be sufficient to predict cardiovascular risk in healthy individuals through the Fuster-BEWAT score. Objective: To compare the results of the Fuster-BEWAT score and the ideal cardiovascular health score to determine health. Method: A cross-sectional descriptive study was carried out in the Primary Care Service of the Hospital Manuel Fajardo Rivero in Santa Clara, from January to June 2019, evaluating the cardiovascular health status of 347 healthy individuals without a history of cardiovascular disease, between the ages of 40 and 59 years old. Cohen's kappa coefficient was used to determine the concordance between the ideal cardiovascular health score and the Fuster-BEWAT score. Results: Low physical activity (79.8%), unbalanced diet (74.9%), and uncontrolled blood pressure were the worst markers found. Only 15.3% of the analyzed men and women met the ideal condition for all markers of the Fuster-BEWAT score and 17.8% of the ideal cardiovascular health score. The concordance obtained between both scores, through Cohen's kappa coefficient (0.935), showed an almost perfect relationship. Conclusions: Both scores showed similar values. The Fuster-BEWAT score is simpler and does not require analytical results; it can be considered as a first option in contexts where access to laboratory tests is limited.

RESUMEN Introducción: La hiperostosis esquelética idiopática difusa (DISH, por sus siglas en inglés) es una afección caracterizada por la calcificación y la osificación progresiva de los ligamentos y las entesis. La mayoría de los pacientes permanecen asintomáticos hasta etapas avanzadas de la enfermedad, donde la limitación y el dolor son característicos. Objetivo: Describir las características demográficas, clínicas y radiológicas de los pacientes con DISH evaluados en el Centro Médico Imbanaco de Cali y en la Clínica de Artritis Temprana, en Cali, Colombia. Materiales y métodos: Es un estudio descriptivo, de corte transversal. Se revisaron los registros de pacientes diagnosticados con DISH, seguidos entre enero de 2000 y octubre de 2018. El diagnóstico se confirmó según los criterios de Resnick-Niwayama. Se encontraron 24 pacientes, todos se incluyeron para el análisis final. Resultados y discusión: En esta serie se encontraron 20 varones y 4 mujeres. La mediana de edad al diagnóstico fue de 70,5 arios (RIQ: 61,3-73,8 arios), siendo menor en las mujeres (71,5 versus 60 años; p = 0,04). La mediana de tiempo de evolución de los síntomas fue de 5 años (RIQ: 3-10 años), la duración fue menor en el grupo de las mujeres (5 versus 4 años; p = 0,20). El 54,2% tenían sobrepeso y el 20,8% eran diabéticos. El síntoma principal fue la limitación cervical. Los segmentos vertebrales (C: cervical; T: torácico; L: lumbar) más afectados por la osificación del ligamento longitudinal anterior (LLA) fueron C5-C6, T8-T10 y L1-L3. La afección periférica predominó en las crestas ilíacas. Todos los pacientes realizaron terapia física y 3 fueron sometidos a cirugía cervical. El grado de limitación funcional fue valorado en 19 pacientes a través de los cuestionarios modified Health Assessment Questionnaire (mHAQ) y Bath Ankylosing Spondylitis Functional Index (BASFI). La mediana del puntaje de ambos cuestionarios fue 2 veces más alta en las mujeres. Conclusión: La DISH fue más frecuente en varones mayores de 65 años y se asoció con enfermedades metabólicas como la obesidad y la diabetes. Aunque las diferencias no fueron significativas, los resultados sugieren que las mujeres presentan un fenotipo grave de la enfermedad explicado por el inicio temprano y curso progresivo de los síntomas, así como mayor limitación funcional medida por mHAQ y BASFI.


ABSTRACT Introduction: Diffuse idiopathic skeletal hyperostosis (DISH) is a condition characterised by calcification and progressive ossification of ligaments and entheses. Most patients remain asymptomatic until advanced stages of the disease, where limitation and pain are characteristic. Objective: To describe the demographic, clinical, and radiological characteristics of patients with DISH evaluated in the Centro Médico Imbanaco and Clínica de Artritis Temprana, in Cali, Colombia. Materials and methods: A descriptive, cross-sectional study was conducted by reviewing the records of patients diagnosed with DISH between January 2000 and October 2018. The diagnosis was confirmed according to the Resnick-Niwayama criteria. A total of 24 patients were found, and all were included for the final analysis. Results and discussion: The series included 20 men and 4 women, with a median age at diagnosis of 70.5 years (IQR 61.3-73.8 years), beinglower in women (71.5 versus 60 years; P=.04). The median time of onset of the symptoms was 5 years (IQR 3-10 years), and the duration was shorter in women (5 versus 4 years; P=.20). It was observed that 54% were overweight and 20% were diabetic. The main symptom was cervical limitation. The most affected vertebral segments due to the ossification of the anterior longitudinal ligament (ALL) were C5-C6, T8-T10 and L1-L3 (cervical C; thoracic T; lumbar L). The peripheral involvement was mainly in the iliac crests. All patients received physiotherapy, and three of them underwent cervical surgery. The degree of functional limitation was assessed in 19 patients using the mHAQ (Modified Health Assessment Questionnaire) and BASFI (Functional Ankylosing Spondylitis Functional (BASFI) questionnaires. The median score of both questionnaires was 2 times higher in women Conclusion: Diffuse idiopathic skeletal hyperostosis was more frequent in men over 65 years of age, and was associated with metabolic conditions such as obesity and diabetes. Although the differences were not significant, the results suggest that women have a more severe phenotype of the disease, explained by the early onset and progressive course of symptoms, as well as greater functional limitation measured by mHAQ and BASFI.

RESUMEN Objetivo Estimar el intervalo serial y el número básico de reproducción de COVID-19 entre casos importados durante la fase de contención en Pereira, Colombia, 2020. Método Se realizó un estudio cuantitativo para determinar algunos aspectos de la dinámica de transmisión de la COVID-19. Se utilizaron las entrevistas epidemiológicas de campo en los que se incluyeron 12 casos confirmados por laboratorio con PCR-RT para SARS-CoV-2 importados y sus correspondientes casos secundarios confirmados, entre los que estaban contactos familiares y sociales. Resultados Los intervalos seriales en la COVID-19 se ajustan a una distribución Gamma, con una media del intervalo serial de 3,8 días (± 2,7) y un R0 de 1,7 (IC 95% 1,06-2,7) inferior a lo encontrado en otras poblaciones con inicio del brote. Conclusiones Un intervalo serial inferior al periodo de incubación como el que se estimó en este estudio sugiere un periodo de transmisión presintomático que, según otras investigaciones, alcanza un pico promedio a los 3,8 días, hecho que sugiere que durante la investigación epidemiológica de campo la búsqueda de contactos estrechos se realice desde al menos 2 días antes del inicio de síntomas del caso inicial.(AU)


ABSTRACT Objective To estimate the serial interval and the basic reproduction number of COVID-19 between imported cases during the containment phase in Pereira-Colombia, 2020. Method A quantitative study was carried out to determine the transmission dynamics for COVID-19. Field epidemiological data were used, which included 12 laboratory-confirmed cases with RT-PCR for imported SARS-CoV-2 and their corresponding confirmed secondary cases, including family and social contacts. Results The serial intervals in COVID-19 fit a Gamma distribution, with a mean of the serial interval of 3.8 days (2.7) and an R0 of 1.7 (95% CI 1.06-2.7) lower than that found in other populations with onset of the outbreak. Conclusions A serial interval lower than the incubation period such as that estimated in this study, suggests a presymptomatic transmission period that according to other investigations reaches an average peak at 3.8 days, suggesting that during the field epidemiological investigation the search for contacts Narrowing is performed from at least 2 days before the onset of symptoms of the initial case.(AU)

ABSTRACT Background: The left atrial appendage (LAAp) resection is an effective treatment approach to reduce the risk of thromboembolism in patients with atrial fibrillation. Objective: To study was to study the impact of removing atrial appendages in the production of natriuretic peptides (NPs) in conditions of volume overload and to develop an experimental model of LAAp resection. Materials and Methods: In a swine model of ischemic heart failure (HF), serum NP levels were measured before (Basal-1A) and after (Basal-1B) a fluid overload. Animals were grouped as follows: (0) preserved appendages, (1) resected LAAp, and (2) both atrial appendages resected. Levels of NP were measured before (2A) and after a fluid overload (2B). Results: Furin levels were higher in Group 0-2A than in Group 2-2A, and a significant increase was found in Group 0-2B compared to Groups 1-2B and 2-2B. Corin levels increased in Basal-1B versus Basal-1A. Atrial NP (ANP) decreased in Basal-1B compared to Basal-1A. After HF induction, ANP increased in Groups 2-2A and 2-2B. Conclusions: Resection of atrial appendages drastically modifies the natriuretic mechanisms of cardiac homeostasis, especially after a fluid overload challenge. Herein, we describe the face and predictive validation of an animal model of atrial appendage resection useful to investigations in translational medicine.

SUMMARY Melatonin has anti-inflammatory and antioxidant properties that can influence tissue growth and apoptosis. This aspect may influence the success of organ transplantation. OBJECTIVE To evaluate the relationship between melatonin and organ transplantation. METHODS A systematic review was performed in PubMed databases using the search terms: "melatonin physiology" or "melatonin therapy" and "transplant pharmacology" or "transplant physiology" or "transplant therapy" or "Transplant therapy". Experiments on the organs of the reproductive system were not included. After analysis, five articles were selected after reading the title and abstract of 50 manuscripts. The works were divided into two aspects: a) analysis of the influence of the organ transplantation procedure on melatonin production; b) action of melatonin on organ transplantation. RESULTS The cardiac transplantation surgical procedure, immunosuppression, and graft did not influence melatonin secretion in rodents, but there was a significant reduction of melatonin in the renal transplantation procedure in patients with renal insufficiency. Melatonin administration in experimental models decreased rejection and improved transplant success. CONCLUSION Studies show that melatonin can reduce organ and species dependence, and the use of melatonin decreases graft rejection.


RESUMO A melatonina tem propriedades anti-inflamatórias e antioxidantes que podem influenciar o crescimento e a apoptose dos tecidos. Esse aspecto pode influenciar o sucesso do transplante de órgãos. OBJETIVO Avaliar a relação entre a melatonina e o transplante de órgãos. MÉTODO A revisão sistemática foi realizada nas bases de dados do PubMed, usando os termos de pesquisa: "fisiologia da melatonina" ou "terapêutica da melatonina" e "farmacologia do transplante" ou "fisiologia do transplante" ou "terapêutica do transplante" ou "terapia do transplante". Não foram incluídos os experimentos sobre os órgãos do sistema reprodutivo. Após análise, cinco artigos foram selecionados após a leitura do título e do resumo de 50 manuscritos. Os trabalhos foram divididos em duas vertentes: a) análise da influência do procedimento de transplante de órgão na produção de melatonina; b) ação da melatonina sobre o transplante de órgãos. RESULTADOS O procedimento cirúrgico do transplante cardíaco, a imunossupressão e o enxerto não influenciaram a secreção de melatonina em roedores, mas houve redução significante da melatonina nos casos do procedimento de transplante renal em pacientes com insuficiência renal. A ministração de melatonina em modelos experimentais diminuiu a rejeição e melhorou o sucesso de transplante. CONCLUSÃO Os estudos mostram que a melatonina pode reduzir a dependência da espécie e do órgão e que o emprego da melatonina diminui a rejeição do órgão.

ABSTRACT

The cryopreservation of sperm and embryos is useful to efficiently archive valuable resources of genetically engineered mice. Till date, more than 60,000 strains of genetically engineered mice have been archived in mouse banks worldwide. Researchers can request for the archived mouse strains for their research projects. The research infrastructure of mouse banks improves the availability of mouse resources, the productivity of research projects, and the reproducibility of animal experiments. Our research team manages the mouse bank at the Center for Animal Resources and Development in Kumamoto University and continuously develops new techniques in mouse reproductive technology to efficiently improve the system of mouse banking. In this review, we introduce the activities of mouse banks and the latest techniques used in mouse reproductive technology.

ABSTRACT

The feeding habits of Callorhinchus callorynchus were investigated in coastal waters off northern Argentina. The effect of body size, seasons and regions was evaluated on female diet composition using a multiple-hypothesis modelling approach. Callorhinchus callorynchus fed mainly on bivalves (55.61% PSIRI), followed by brachyuran crabs (10.62% PSIRI) and isopods (10.13% PSIRI). Callorhinchus callorynchus females showed changes in the diet composition with increasing body size and also between seasons and regions. Further, this species is able to consume larger bivalves as it grows. Trophic level was 3.15, characterizing it as a secondary consumer. We conclude that C. callorynchus showed a behavior of crushing hard prey, mainly on bivalves, brachyuran, gastropods and anomuran crabs. Females of this species shift their diet with increasing body size and in response to seasonal and regional changes in prey abundance or distribution.(AU)


RESUMEN Los hábitos alimentarios de Callorhinchus callorynchus fueron investigados en las aguas costeras del norte de Argentina. Se evaluó el efecto del tamaño del cuerpo, la temporada y la región sobre la composición de la dieta de las hembras mediante un enfoque de modelado de múltiples hipótesis. Callorhinchus callorynchus se alimentó principalmente de bivalvos (55,61% PSIRI), seguido de cangrejos brachyuras (10,62% PSIRI) y de isópodos (10,13% PSIRI). Las hembras de C. callorynchus presentaron cambios en la dieta con incremento del tamaño del cuerpo, la temporada y la región. Además, esta especie es capaz de consumir bivalvos de mayor tamaño a medida que incrementa el tamaño del cuerpo. El nivel trófico fue calculado en 3,15, caracterizando a esta especie como un consumidor secundario. Concluimos que C. callorynchus presentó un comportamiento de triturador de presas duras, principalmente bivalvos, cangrejos y gasterópodos. Las hembras de esta especie cambian su dieta con el incremento del tamaño del cuerpo y en respuesta a cambios temporales y regionales en la abundancia y distribución de sus presas.(AU)

ABSTRACT

Abstract We report field infestation under natural conditions by the cacao plum moth, Michaelophorus nubilus (Felder & Rogenhofer) on leaves of cacao (Theobroma cacao L.; Malvaceae) for the first time in Southern Chiapas, Mexico. The larvae of M. nubilus damages new "flushing" or light green leaves from buds, from either the main or lateral branches of cacao plants.
